DS-11 ADULT/MINOR PACKAGES

This form is used to apply for a U.S. passport book and/or card in person at an acceptance facility, a passport agency (by appointment only), or a U.S. embassy, consulate, or consular agency (if abroad). 

To be eligible to use this form you must apply in person if at least one of the following is true: 

I am applying for my first U.S. passport I am under age of 16 

My previous U.S. passport was either: 

a) issued under age 16; 

b) issued more than 15 years ago; 

c) lost, stolen, or damaged  

What To Bring 

1. PROOF OF U.S. CITIZENSHIP Information can be found at travel.state.gov/citizenship.  

2. PROOF OF IDENTITY Information can be found at travel.state.gov/identification 

3. A RECENT COLOR PHOTOGRAPH See the full list of photo requirements on travel.state.gov/photos 4. Payment for passport fees(if we are providing the check) and GET THISS PASSPORT fees 

5. Do not bring the $35 acceptance fee. That is paid in cash directly to the passport acceptance agent at your passport appointment with an approved facility.

The Review Service  package covers checking for accuracy, errors, completeness, passport photo compliance, and the most common application mistakes. This service is designed to help reduce avoidable errors and corrections, issues caused by missing or incomplete information, insufficient documentation, or noncompliant passport photos—all of which may delay processing or prompt the U.S. Department of State to request additional information or documentation.

Please note: 

Applications MUST be completed before your scheduled appointment time. If not, you will be asked to reschedule for the correct service type and availability. No exceptions!

Please have your passport photos, original or certified copies of your documents, and passport book and/or card (if applicable) with you at your appointment.

The Premium Service  package includes everything in the Review Service Packages plus application completion, passport application assembly, passport photos, copies, notary services, a check for passport fees, and administrative processing fees. This service is designed to help reduce avoidable errors and corrections, unnecessary time spent at a Passport Acceptance Facility or Passport Agency, and issues caused by missing or incomplete information, insufficient documentation, or noncompliant passport photos—all of which may delay processing or prompt the U.S. Department of State to request additional information or documentation.

This package covers 1 applicant only and includes passport photos.

The Family Service process includes everything in the Review and Premium Service Packages and covers up to 5 applicants. This service is designed to help reduce avoidable errors and corrections, unnecessary time spent at a Passport Acceptance Facility or Passport Agency, and issues caused by missing or incomplete information, insufficient documentation, or noncompliant passport photos—all of which may delay processing or prompt the U.S. Department of State to request additional information or documentation.

This package covers up to 5 applicants and includes passport photos.

The Corporate Service process includes everything in the Review and Premium Service Packages and covers up to 8 to 12 applicants. This service is designed to help reduce avoidable errors and corrections, unnecessary time spent at a Passport Acceptance Facility or Passport Agency, and issues caused by missing or incomplete information, insufficient documentation, or noncompliant passport photos—all of which may delay processing or prompt the U.S. Department of State to request additional information or documentation.

This package starts at a minimum of 8 applicants but no more than 12. Photos are also included.
