This form is used to apply for a U.S. passport book and/or card in person at an acceptance facility, a passport agency (by appointment only), or a U.S.
embassy, consulate, or consular agency (if abroad).
To be eligible to use this form you must apply in person if at least one of the following is true:
This form is used by U.S. passport holders to renew their current or recently expired U.S. passport book and/or card (a travel document attesting to one’s identity and issued to U.S. citizens or non-citizen U.S. nationals).
This form can be submitted by mail.
You are eligible to use this form if you can answer “YES” to ALL statements below about your passport:
1. I can submit my most recent U.S. passport book and/or card with this application.
2. I was at least 16 years old when my most recent U.S. passport book and/or card was issued.
3. I was issued my most recent U.S passport book and/or card less than 15 years ago.
4. The U.S. passport book and/or card that I am renewing has not been mutilated, damaged, or reported lost or stolen.
5. My U.S. passport was not limited to less than the normal ten-year validity period due to passport damage/mutilation, multiple passport thefts/losses, or non-compliance with 22 C.F.R. 51.41. (Refer to the last page of your U.S. passport book for endorsement information.)
6. My name has not changed since my most recent U.S. passport book and/or card was issued.

My name has changed by marriage or court order, and I can submit proper certified documentation to reflect my name change (such as a certified copy of a marriage certificate, or court order).
If you answered "NO" to any of the statements above, STOP. You “CANNOT” use this form. Please use the DS-11 or DS-5504 form depending on your circumstances.

Any U.S. citizen traveling internationally by air must have a valid U.S. passport to enter or re-enter the United States.
Minors age 16 or younger may present proof of U.S. citizenship, such as an original or certified birth certificate, when returning from Canada or Mexico by land.
A passport is not required for travelers on a closed-loop cruise (a cruise that begins and ends at the same U.S. port), though it is highly recommended in case of emergency travel or unexpected itinerary changes.
You cannot travel with an expired passport.
Your passport must be valid for at least six months beyond the date you plan to return to the United States.
If your passport has expired—or will expire within that timeframe—you’ll need to renew it before traveling to avoid delays or denied entry.
Under the Western Hemisphere Travel Initiative (WHTI), all travelers — including infants — are required to have a valid U.S. passport when traveling on a cruise that is not a closed-loop cruise (a cruise that begins and ends at the same U.S. port).
A closed-loop cruise is a voyage that departs from and returns to the same U.S. port. Most cruises that begin and end in the same location are considered closed-loop, meaning they start and finish at the same U.S. port and travel only within the Western Hemisphere.
For example, if you board a cruise ship in Fort Lauderdale, Florida, visit at least one foreign port of call such as Bermuda or Cancún, and then return to Fort Lauderdale, your trip qualifies as a closed-loop cruise.
However, if you board a cruise in San Diego, California, sail through the Panama Canal (stopping at a foreign port), and end the cruise in Miami, Florida, it is not considered a closed-loop cruise.

A visa is a document that allows you to enter a foreign country for a specific purpose and length of time.
No, not all countries require a visa for travel. It depends on your destination, length of stay, and purpose of travel.
If changes are being made within one year from the date your passport was issued you will need to complete Form DS-5504 without any fees. If you’re making the change more than one year from the issue date you will need to complete and submit the DS-82 and pay all applicable fees.

You must submit a completed attestation form with your passport application
identifying yourself as a class member and what sex marker you want in your passport.
If you are selecting M or F, select it on the attestation form and application. If you are selecting X, leave the sex field blank on your application and select it on the attestation.
