ds-5504 DATA CORRECTION, NAME CHANGE, LIMITED-VISIBILITY PASSPORT FORM

This form is used by U.S. passport holders who are eligible to re-apply for a U.S. passport book and/or card at no cost (unless expedited service is requested for a name change or limited passport replacement) if you meet certain criteria.

You are eligible to use this form if you can answer “Yes” to any of the statements below about your passport: 

I changed my name, by marriage or court order, less than one year after my most recent U.S. passport book and/or card was issued and my U.S. passport book and/or card was issued less than one year ago. 

-- OR -- 

My identifying information in my most recent U.S. passport book and/or card was printed incorrectly. 

-- OR -- 

My most recent U.S. passport book was obtained at the full passport fee and limited to two years or less for a reason other than multiple losses or serious damage/mutilation of previous passports. 

What To Bring

1.     YOUR MOST RECENTLY ISSUED U.S. PASSPORT BOOK AND/OR CARD

2.     PROOF OF NAME CHANGE: must have occurred within 1 year or less of when your most recent passport book and/or card was issued.

CORRECTION: If there is a data or printing error 

3.     A RECENT COLOR PHOTOGRAPH (we take photos on-site)

4.     EXPEDITE FEE (Optional)

5.     Payment for passport fees(if we are providing the check) and GET THISS PASSPORT fees

Please come with your original or certified copies of your documents and most recently issued passport book and/or card.

The Review Service package covers checking for accuracy, errors, completeness, passport photo requirements, and the most common application mistakes. This service is designed to help reduce avoidable errors and corrections by identifying missing or incomplete information, insufficient documentation, or noncompliant passport photos—all of which may delay processing or prompt the U.S. Department of State to request additional information or documentation.

Applications MUST be completed before your scheduled appointment time. If not, you will be asked to reschedule for the correct service type and availability. No exceptions!

This package covers 1 applicant only and does not include passport photos.

Please have your most recently issued passport book and/or passport card and original or certified copies of your documents with you at your appointment.

The Premium Service package includes everything in the Review Service Package plus completion, application assembly,  photos, notary,  and mailing your application via USPS Priority Mail. 

Overnight mail is also included if you are expediting your passport.

This package covers 1 applicant only and includes passport photos.
