DS-82 ADULT ONLY RENEWAL PACKAGES

This form is used by U.S. passport holders to renew their current or recently expired U.S. passport book and/or card (a travel document attesting to one’s identity and issued to U.S. citizens or non-citizen U.S. nationals). This form can be submitted by mail. 

You are eligible to use this form if you can answer “YES” to ALL statements below about your passport: 

1.     I can submit my most recent U.S. passport book and/or card with this application.

2.     I was at least 16 years old when my most recent U.S. passport book and/or card was issued. 

3.     I was issued my most recent U.S passport book and/or card less than 15 years ago. 

4.     The U.S. passport book and/or card that I am renewing has not been mutilated, damaged, or reported lost or stolen. 

5.     My U.S. passport was not limited to less than the normal ten-year validity period due to passport damage/mutilation, multiple passport thefts/losses, or non-compliance with 22 C.F.R. 51.41. (Refer to the last page of your U.S. passport book for endorsement information.) 

6.     My name has not changed since my most recent U.S. passport book and/or card was issued. 

-- OR -- 

My name has changed by marriage or court order, and I can submit proper certified documentation to reflect my name change (such as a certified copy of a marriage certificate, or court order). 

If you answered "NO" to any of the statements above, STOP. You “CANNOT”  use this form.  Please use the DS-11 or DS-5504 form depending on your circumstances.

What To Bring

1.     YOUR MOST RECENTLY ISSUED U.S. PASSPORT BOOK AND/OR CARD

2.     PROOF OF NAME CHANGE: if applicable

3.     A RECENT COLOR PHOTOGRAPH (we take photos on site)

4.     Payment for passport fees(if we are providing the check) and GET THISS PASSPORT fees

Please come with your original or certified copies of your documents and most recently issued passport book and/or card. The review process covers checking for accuracy, errors, completeness, passport photo compliance, and the most common mistakes made on applications. This service will reduce the possibility of your application being rejected or delayed by the U.S. Department of State.

The Review Service package covers checking for accuracy, errors, completeness, passport photo compliance, and the most common application mistakes. This service is designed to help reduce avoidable errors and corrections by idenitfying missing or incomplete information, insufficient documentation, or noncompliant passport photos—all of which may delay processing or prompt the U.S. Department of State to request additional information or documentation.

This package covers 1 applicant only and does not include passport photos.

The Premium Service  package includes everything in the Review Service Package plus completion, application assembly,  photos, notary,  and mailing your application via USPS Priority Mail

Overnight mail is optional and will cost extra. Rates vary based on the delivery carrier used.

This is the everything-done-for-you package!

This package covers 1 applicant only and includes passport photos.

Please have your current/expired passport book and/or passport card and original or certified copies of your documents with you at your appointment.

The Family Service package includes everything in the Review and Premium package and covers 3 to 6 adults.

Overnight mail is also included if you are expediting your passports.

This package covers a minimum of 3 and a max of up to 6 ADULT applicants and includes passport photos

3 ADULTS - $180

4 ADULTS - $240

5 ADULTS - $300

6 ADULTS - $360

The Corporate Service package includes everything in the Review and Premium package and covers 8 to 12 applicants.

Overnight mail is also included if you are expediting your passport.

This package starts at a minimum of 8 applicants but no more than 12. Photos are also included.
